With women at the crossroads of the HIV epidemic, El-Sadr provided the keynote speech, reminding participants of unresolved issues. El-Sadr explained that the HIV epidemic has motivated a transformation of the health system in sub-Saharan Africa and challenged the usual paradigm for delivering health care beyond episodic care to include a full continuum of care. She discussed lessons learned, noting HIV has forced the global health community to think beyond service delivery to include the work force, information systems, prevention, family planning and access and equity. She concluded that while global health discoveries sit on bookshelves and in medical journals, we need to move more efficiently into implementation.
